OBSERVATIONS

The patient presented with multiple erythematous plaques on the extensor surfaces of the arms and legs, accompanied by silvery scales. The lesions were well-defined, with some areas exhibiting fissuring. The patient reported pruritus and discomfort, particularly after exposure to irritants. No signs of secondary infection were noted. The patient has a family history of psoriasis and has experienced similar symptoms intermittently over the past few years.

ASSESSMENT

Based on the clinical presentation and history, the findings are consistent with plaque psoriasis. The distribution and characteristics of the lesions align with typical manifestations of this condition. Differential diagnoses considered include seborrheic dermatitis and eczema; however, the distinct plaque formation and scaling favor psoriasis as the primary diagnosis.

SEVERITY

The severity of the condition is classified as moderate. The affected body surface area (BSA) is approximately 10%, and the patient reports a significant impact on quality of life due to itching and visibility of the lesions. The Psoriasis Area and Severity Index (PASI) score calculated is 8, indicating moderate disease severity.

IMMEDIATE CARE

The patient was advised to initiate topical treatment with high-potency corticosteroids to reduce inflammation and pruritus. Additionally, a vitamin D analog was recommended to promote skin cell turnover. The importance of maintaining skin hydration was emphasized, and the patient was instructed to use emollients regularly. Follow-up is scheduled in four weeks to assess treatment efficacy and make necessary adjustments. The patient was also educated on lifestyle modifications, including stress management and avoiding known triggers.
